What should be done first!!

Hello Experts,
Just wanted to clarify a small doubt.
If a stakeholder request for a change to modify the scope slightly to add a feature. Project is in execution phase. What should be te response of the PM ?
Choices:
Analyze the impact of the request on the other factors? OR
Document the change request as per project scope management plan and then do a analysis.
On a side note : I have seen questions where analyze the impact is been choosen as a first choice.
My ask is : What is PMI theory on this.?

Another small boubt :
Q:Lessons learned created by which of the following (a questions from exam central)
Choices :
Stakeholder
Project team
Opted stakeholder however the right choice was given as project team..
Earlier mock i attempted, stakeholder was selected as a right choice. Not sure what to select in case the same question appears in the PMP actual exam. :)
Pl suggest.
