PM Experiences Document To register for Exam -- Any Template Document to share ?

Guys,

I will attend the PMBOK5 Exam soon..

Regarding the expériences that we have to show to PMI to be accept for the Exam.

I would like to prepare an offline document ( Excel, Word )

I was wondering if anyone can share a Template document to help me fill my PM Experience that is required to do the Exam.

I would prefer to fill that offline before subscribe to the web portal..
