Since the Change request has been approved and added to your project plan you will now need to execute the same.
A is not correct since there are no defects reported here.
C is not correct since CR has already been initiated and approved
D is not correct since its not only about the future road quality. its about the road already laid out. Corrective action means something which is done and you are now correcting. The future road will be laid out as per the revised project management plan so there is no corrective action here.

C & D are not correct, but why is A not correct? Please advise.
My thoughts: The defect(s) is/are the road micro texture quality not being in line with the quality levels specified in the PM Plan. As per the question, many change requests for the rectification of this issue have been submitted by stakeholders and appproved by CCB. Hence, it is clear that rework needs to be done - how much we don't know. But rework, yes for sure.
Since the project execution phase is still going on, end users will not be using the road, and so I am not sure what 'negative consequences associated with the poor quality of the road' (in choice B) implies.
In my mind, the idea is to bring the road quality up to the laid down standard prior to project closure and prior to road usage. So answer choice A should be correct??
Thoughts?

Defect is when the road is
Defect is when the road is laid out and it does not meet the specifications. Here the specifications where changed using change management procedure, so they have to re-do the road, which was laid out earlier.
If you are painting your house , you select black color then you find out that black is making my house dull and you change the color to white. The black paint is not defect.

I respectfully disagree with the explanation. Here's why:
Nowhere is it mentioned that the specifications were changed. The said change requests were intended to "bring the quality of the micro texture of the road aligned with the quality levels incorporated in the project management plan."
The micro texture of the actual new road was found - most probably during QC inspection - to be below the quality levels already specified in the PM Plan. Hence rework is needed to upgrade the road quality so that it aligns/matches with what is already specified in the PM Plan. The change requests are for the required rework (and not for changing the specifications). That is how I would interpret the question.
