 that is what I choose but they said is C all this mock is confusing now

 I agree it's C. You must first look at the impact of the change on cost, schedule, etc. Once  you look at the impact, then you can initiate the change request. Looking at the impact first can allow you to go back to the customer (before u create a change request) and let them know the type of impact it is going to make. At this point, they may say they don't wanna do it anymore.

 

Nish, the question says that the PM has already looked at the impact on triple constraints. Per your post then the answer should be A (issue change request)?

The tricky about this quetion is it is talking about assessing the triple constraints but actually we need to asses the other constrainst as well like Quality, Risk, Customer satisfaction. Purposely he has mentioned about the triple constriant (Cost, scope and time) but there are other things need to be taken care like above when the specifics are given as option. Like in this it has Quality objectives and Quality planning. if this wasn't there as an option then the answer could be different.

So considering the above explanation from me I would go for option C.
