The trap in this question is breaking down the lowest level of WBS ( i.e Work Packages)

In Work packages, we define the activities and hence the answer is A.

regarding Q1, it was mentioned that high level scope is only defined, hence we can consider that collect requirements and define scope is not completed. Please correct me if I'm wrong

Q1 - A. Create WBS

If we see the Pmbok definition of,  Define scope - Process of developing a detailed description of the project and product. Otherwise called as detailed scope.
It implies that we can consider Collect requirements as high level scope though nowhere indicated like that
 
In the order the previous one's output goes as input to the next,
  1. Collect requirements - Requirements documentation
  2. Define scope - Detailed scope, Project scope statement
  3. Create WBS - Scope baseline
  4. Define activities - Activity list
Since Define scope is not listed as a choice, so the team had forgotten "Create WBS" process and moved to Define activities
 
Q2 - A. Define activities
Lowest level components of WBS is a work package. We use Decomposition technique to breakdown each work package into activity list

High level scope means constructing a Building, let us say

Define list of activites- Activities  can be painting, concreting, plastering etc. can only be listed after the workpackages are created

He has directly gone from Building to Define list of activities without touching the collect requirements,  Define scope and WBS etc.

Since the Answers listed is WBS, the option is WBS.

The high­level scope is defned and documented within the project charter and is based
on the business and compliance requirements.

 Q1 is not relevant to pmbok 4, the term "high level scope" is a term of pmbok 3 (refer p 375 of pmbok 4)

Technically question is not fitting with its content and options in light of PMBOK4 processes and activities sequences.

Q2 - is quite obvious - option A is the answer. 

As PMBOK 4 says "define activities" is a further decomposition of wpkgs.

Q is asking for a process, not for techniques.
