Can some body help me to fill my PMI form

1. There are multiple Project role such as (Project Contributor, Supervisor, Manager, Project leader, Project manager ,Educator, Consultant, Administrator, Other). Can You please map these roles according to Phase like Initiating, Planning , execution , Monitoring and control then Closing. Like Under Initiation, what role will come etc.


 


 


2. Can I put more than 4500 Hours in PMI form ?
