Plan Quality, Perform QA and QC...

Apropos to a query posted by Javerose on 10/7/2010 (long back) on one of the most troubling topics of PMP and that is QC and QA. Let me start with his/her question first,
Hi Admin, thanks for your reply, now is more clear.
Look the following question (I know should be post it in a different thread) but is related to QA and QC
Joe is a project manager on an industrial design project. He has found a pattern of defects occurring in all of his projects over the past few years and he thinks there might be a problem in the process his company is using that is causing it. He uses Ishikawa diagrams to come up with the root cause for this trend over projects so that he can make recommendations for process changes to avoid this problem in the future. What process is he doing?
A. Plan Quality
B. Perform Quality Assurance
C. Perform Quality Control
D. Qualitative Risk Analysis
The answer is "B" and he raises question WHY? When I read the question I also realised this is QC but then later realised nopes, its QA. I will tell why?
The PM is doing process improvement which is done in Perform QC process and, moreover, he is using the T&T of perform QC (Ishikawa) and actually Ts & Ts of Plan quality and Perform QC is actually a Perform QA tool. This is what I feel.
Any EJ would be more than welcome!

My first impression on seeing the oprions is "Quality Assurance".
The reason being, Joe had observed a patterrn of defects and he feels there is a problem with the process(ie Quality assurance). Since he is addressing the problem with the process and using the Ishikawa to fine th root cause of the probem,(this is a red herring statement as far as the question is concerned) this belongs to QA. QC is concerned with the actual testing of the end product before the verify scope process is traiggered.
Bottomline:
QA is cocnerned with the processed and idenifying/rectifying process flaws
QC is concerned with the actaul testing of the application
Any other suggestions /opinions are welcome

Right
Yes, right Karthik...
And noteworthy point is that Quality control measurements are one of the 4 inputs to Perform QA, and the other 3 being PMP, WPI, and Quality Metrics.
