You are...
Shashiji,
You are asking for help to write on your project details which, incidentally, was done only by you. What help you need in this.
Write all your project data or what you did in initiation, -planning, M&C et al. What can somebody else tell you on this? That is why I wrote you have to write in Angrezi (English) and then everything will be scribbled by you and only you pal.
When you sit in the prometric to take the PMP test, you would see many opportunities (questions) where you will have to go by your own judgment and that is the time no zillians will help you, not even SS or Saket Sir.
Hope you fill the form soon and do well partner!
All the best!!!
Warmly, KK....

Never...
Shashiji,
How on earth did you get this WONDERFUL idea that I am furious? Why would I be? You pass or fail, or I pass or fail does it bother even an inch to either of us. I feel none whatsoever.
Sir, when I started preparing few days back, I asked a question, "How come deliverables be an ouput of DMPE. Should it not be a part of MCPW only since that is where deliverables are being checked for their verifiability?"
I received an answer from Mr Saket, whom I affectionately call SS or Saket Sir. He said can MCPW do anything if DMPE is not giving deliverables to them? I understood. Wow!!! If I would have applied some of brain, which is very scanty anyway, I would have got the answer myself rather than me making a mockery of self and disturbing others. Since then whenever I see a question where, I assume, the questions has been asked JLT or Just Like That, I immediately barge in.
PMP takes a heavy toll on oneself pal. I failed once. Why? I was in a habit of spoon-feeding and that is why I did not learn or unlearn much and I do not want anyone to go through the same way as I did.
Last but not the least, I beg for your forgiveness Shashiji, if I was construed to be alarmingly dishonouring someone's dignity. Kindly accept my heart-felt apologies Shashiji.
Warmly, KK...

Try to use project management...
Try to use project management words to describe your role in each project, something like.
I played the role of a Team Leader. My roles were to interact with the project stakeholders, understand the business requirement. Create high level and low level documents for the business requirement with my team and get it approved. Get the deliverables complete my team. Participated in quality management like audits and quality control. Follow the chance request process. Etc etc
Something like this you should be able to draft as per your role and involvement in the project work. 6-7 lines should be sufficient for each project.
