Change requests and conflict of interests

Hi friends,
I am stuck at two different questions. The first one is related to Conflict of Interest.
1. In a conflict of interest scenario, is it recommended to inform the concerned management first and then proceed with the action or avoid conflict of interest situation in all cases.
2. When requesting change requests, do they need to be formal and written or can they be verbal too. If verbal is accepted, what are the circumstances.
I would really appreciate detailed answers as these two are confusing me a lot

Response to Q1: In a Conflict in Interest scenario, Yes You have to first intimate the appropiate authority regarding your concealed interest (involvement) in any form and it is best to separate yourself from that particular activity.
Response to Q2: Requested Changes have to be formal and written with alternatve analysis and clear evaluation of impacts on constraints. Then it has to proceed thru CCB. Verbal Change requests should never be prescribed to anybody.
