Kickoff meeting happens at the beginning of the execution phase. When I took the PMP 35 hour contact with PMI, the instructor insisted on getting to know that as it maybe a question on the exam.
Kickoff is different from kickoff meeting. Generally we say that during initiation phase the project is kicked off, but kickoff meeting can happen after team has been acquired
As far as I know, the kick-off meeting should happen at some point in the Planning stage; the project mgmt plan may or may not be complete at that time. But the meeting should definitely be conducted before start of Execution of work packages.
