My answer is B Analyze the impact of the request on the other factors of triple constraints namely cost and schedule but it is wrong can anyone please explain why option D is correct ?

Document it first
First of all the change request must be documented. It can not be in a verbal form. Once the request is documented it can be analyzed for its impact on other areas. Then the change request with it's impact analysis should be sent to change control board for approval.

As per pmbok p94, change request may be initiated by any of stakeholder verbally, but before analysing anything, it is natural, you will have to register (document) it .
