PMP Audits

I was choose for PMP Audits


In the PMP Audits section, it says:


"After they verify, they will put the completed Experience Verification Form and


Experience Record in an envelope, sign his/her name over the sealed flap of the


envelope, and return the envelope to you via in person or by mail"


The manager must fill ONLY the PDF document call "Form" or he has to create another
document?  What means "Experience Record?" Is it a free document with a resume of my work in the project?


The manager has to make two signs? One on the form and another on the envelope?

as far as i know, the Manager needs to sign the FORM (pdf); place this form in an envelop and cross sign over the seal flap along with his name & contact number.


In case of multiple projects that needs to be signed by same manager, he may create "Experience Record" document (this document is a summary of all the projects you have worked under his supervision) and sign that document. By doing this, he need not to sign on each form for individual project. But it depends on you, to request your manager to sign on individual form OR create summary sheet & then sign for multiple projects. But make sure you & your manager are very clear on the objective of this verification.
