PMP- Question on Change request processing sequence ?

Hi,

Q1. From the books and tesks taken, I couldnt come to a conclusion on the change management sequence. This is what I think of. Folks, who has done it as per PMP process, please correct the the step sequence below.  I feel it will help many out there.

1. The M&C process shows a variance/a defect is identified/a new request coming from  stakeholder

2. PM will analyze the impact of the change along with the team, against 6 project constraints

3. If the change is within PM authority (or CCB not available in org) -> CR is generated, doesnt go to ICC or CCB - >CR will be appoved by PM ->The change will be entered into change log after implementation -> Communicate the change to stakeholders [somehow].

4. If the change is beyond PMs control, will go for CR approval process.  as below

5. If the request is from a stakeholder, PM will inform the impacts to the stakeolder -> PM will request them to create a formal CR.

6. The CR goes to the CCB and gets approval/rejection  from the board which may include the customer representative.

7. Approved CRs goes to execution process DMPE -> The change will be entered into change log after implementation -> Communicate the change to stakeholders.

------------------------------------------------------------------------------------------------------------

Q2. Step 2, when should change log be updated ? before or after the implentation ?

Q3. Why do we need a CR process also  for implementing the changes which are already pre-planned as risk response strategies in the planning process. ? Isnt it an overhead ? Cant PM bypass this as per step 3 ?

Q4. Page 312 PMBOK says  Sometimes CR are not needed for implementing contingency plans and work arounds. When this happens, how will the execution process gets the required change input as [the Approved CR ] doesnt comes into picture ?

Q5. If changes doesnt require CR, then will Integrated change control come to picture?.  Then how this change will be tracked, how it will be communicated to stakeholders which I belive happens as part of configuration management system in Integ CControl?

Q6. Crucial question is , do we need CR to be there, to make any project/product change, always ?

I want to get this very clear, as it confuses me a lot in answering the questions.Pls help.
