What should get high priority when beginning a project: effective team creation of establishing firm project requirements?

I was working on some question-answers from a paper we got from PMP course.

The question and its answers are presented below.

I chose answer D - establishing firm project requirements is high-priority when beginning a project.

However, the correct answer is pointed as A - creating an effectiv team.

Even after readin the explanation, I still believe that answer D is correct.

Can you help me understand if I am wrong or the paper is wrong?

Good catch... I am not sure why A would be the right answer, Even B can be correct.
