Observation on ITTOs...
Dear All, Saket Sir, and respected Pawarji Sir,
Please allow me to put something on ITTOs. It might sound very common, however, it might be okay for beginners and failures like yours truly.
POINTS:
1. OPA or Org Process Act is used in all processes of Integration, Cost, Time, and Communication.
2. EJ or Expert Judgment is used in NONE of the processes of Quality and HR.
3. Pro Mgmnt Plan or PMP is used in NONE of any of the PLANNING processes which is 20 in numbers.
Now my next pointers would be, if any, on OPA updates and Project Documents.
EEF updates are ONLY used in two processes of HR:
(A) Develop Project Team (PT), and
(B) Manage PT.
With the wishes of people like Saket Sir, I want to be 100% secure on ITTOs as I remember Saket Sir saying 3-4 days back that almost 80% questions are based out of ITTOs in one way or the other.
My next target after ITTOs would be Formulas.

You are right for everything except
1. You are right for everything except (1), PMBOK says - Updating and adding to the organizational process assets as necessary throughout the project are generally the responsibility of the project team members.
Catch here - Organizational process assets are inputs and outputs of all project phases even if PMBOK do not mention it explicitly. Why should they mention this again and again having said the statement above? If you understand this, you can easily answer 3-4 PMP equation in the actual exam.
I hope you will change your point (1) now.

True, because Expert Judgment is like asking a expert to help you by his experience (its like an expert guess). Quality planning and HR (resources) are project demand based; it cannot be trusted and designed with some expert guess work.
3. Pro Mgmnt Plan or PMP is used in NONE of any of the PLANNING processes which is 20 in numbers.
Right because - The planning processes develop the project management plan and the project documents that will be used to carry out the project. How can you use the output you are making as an input…common sense!

A process is a set of interrelated actions and activities performed to achieve a pre-specified product, result, or service. Each process is characterized by its inputs, the tools and techniques that can be applied, and the resulting outputs. As explained in Chapters 1 and 2, the project manager must consider organizational process assets and enterprise environmental factors. These must be taken into account for every process, even if they are not explicitly listed as inputs in the process specification. Organizational process assets provide guidelines and criteria for tailoring the organization's processes to the specific needs of the project. Enterprise environmental factors may constrain the project management options.
