The way I look at it, if its not in writing. I dont see it.
How do I justify my actions later on if I dont have any written documents to support it.
This will also prevent any confusion or misunderstanding as the request is in black and white.
Anyone else???

D looks correct to me
When you add a feature to the existing design, it means that it will impact the scope baseline and in any impact on scope baseline should first start with Change Control who will decide whether to accept it or reject it. If the CR is approved, then the Project manager can look for impacts and reschedule the project.

No doubt, D is correct and the best answer
Hi dear samir, there is no doubt that the correct answer is D as indicated in that practice exam, and this is the best answer. Let me explain for clarity.
1. First, understand the context of the question. The context is about change request. Though, there are some phrase and situation in the question included to mislead you to think that the context is about ethics, but NO.. the context is about Change Request. Thus, choice A is definitely WRONG.
2. Even if the person is influential, we can listen to him, but not to immediately ADD the functionality of what he wants. Since he is speaking of change in the scope, it has to be documented. Thus, choice B is wrong.
3. Choice C may be misleading to think this is correct. But look, is it logical to proceed with assessing the impact of a verbal demand to change the scope before having the decision? Now, decision to what? Yes, in some cases, even if the request is yet verbal, we can proceed with assessing its impact parallel to the change process, but what makes this choice C blurd is that - decision to what? this means this choice is not clear and incomplete. Thus, it cannot be correct.
4. Choice D is obviously correct. Explanation already written by Admin.
