Project team has delivered its first milestone consisting of two management reports for the HR department. Head of the department compliments the reports but asks how he can drill down into the details of the summary lines. What did you and your team most likely neglect to do ?
1. Conduct stakeholder interviews
2. Categorize the requirements into project vs product.
3. Manage Stakeholder expectations
4. Elict requirements in enough details to be measured as the project progressed.
Can someone help with above Qs ?
The answr given is 4.

they were unable to get
they were unable to get cause ( supporting report) of summary report .
because you have been forgot to add detailed supporting measurement sheets.

Though it is possible that the HOD (one of the stakeholders) was not interviewed, it is more probable that the requirement was not detailed properly. If it were not so, the error would have been caught during the quality control process i.e. measured and controlled as the project progressed.
