A) Makes certain the discoveries are included in the project lessons learned

B) Makes certain the discoveries are reported in the monthly status report

C) Makes certain you mentioned the discoveries at the senior management meeting in two months

D) Makes certain you tell the other project managers involved in this program about the discoveries at the weekly meeting

because it may affect as a risk/opportunity to the other part of program.

other options are time bounded not having any significant impact due to this significant  discovery (identified risk)

I would say D as well for the following reasons:

1.  Choice A is reactive, not proactive

2.  Fellow project managers are best intended audience initally.

3.  Weekly meeting is first opportunity to formally update others.  Waiting a month or two months could significantly affect cost and schedule if/when things are changed.
