A mistake made by a team member on the job
A change that the team needs to make in how they do the work

i guess it is A
A mistake made by a team member on the job---- so anything performed away from the Prj mgmt plan is a defect liek which is not build as per plan and deviates from prj mgmt plan is a defect.

Generally one would think that A is the right answer. But A would have been right if mistake made by team member would have resulted in not meeting the stated requirements. This is a generic statement , which in the light of Project management is perceived as mistake = defect, which may not be true.
C is right because it states that PM plan did not meet requirements.
