Need clarification for Oliver Lehman question

Came across this Question in Oliver Lehman.

Projects frequently do not meet customer expectations for which of the following reasons? 
o Poor risk management by the contractor running the project
o Poor technical ability of the contractor running the project for the customer
o Technical inability and poor risk management by the contractor
o Unclear scope definition by the customer for whom the project is performed
 
Ans given is c, but as per my understanding ans should have been d. Should it not be that Projects fail mostly due to unclear Scope.
Experts Kindly give your valuable feedback.

The question is very specific to industry, I doubt if we can generalized that unclear scope is cause of maximum failures

C is more logical,  Techncial abilities and poor risk mangament will surely lead to failures.
