Help me with this Activity question please

All activities are going as per plan and team members are working as per the work authorization system. However one of the team members who was to start work on his activity today finds tat he has not recieved the necessary inputs from another team member who is working on predecessor activity, On contacting the other team member he comes to know that there will be a delay of three days. What could have most likely happened?
a) Functional Manager was not part of the communication managmenet plan
b) The project manager was focusing on meeting the project deadline
c) Activity sequence was not define in the project schedule
d) All the required stakeholders were not invited to the project status meeting.

I agree with your comment about functional manager managing resources.
In my understanding in matrix org, both functional and project manager have the role of managing the project resources and getting the work done efficiently. Correct me if I 'm wrong here.

a) it is bad, but functional manager doesn't monitor project and doesn't solicit or delivers updates to all team members, each member may have a different functional manager
b)may be - he was so focused on deadline that he forgot to monitor other scheduled activities. But not likely.
c) if activity has a predecessor then sequence is defined
d)if these members were not invited - they did not exchange information about delay. But it means that project manager doesn't collect work performance data and doesn't control schedule. Hmmmm. Should not be the case...

I would think the answer is C. Discarded A as There is not much mentioned abt the org being functional or projectized. Option D is more of a hypothetical situation.

This question is related to schedule management.
Since PM was focussing on meeting the project deadline, he did not consider to include any lags or buffer when creating a schedule.
